Authority to act form

Use this form if you need to act on behalf of a member. For example, if you are the guardian of a member under the age of 18. Only a guardian or family member can be authorised to cancel a policy, remove members, or reduce cover duration.

Accident/injury/
condition form

This form is used so CBHS International Health can assess your claim in relation to an accident, injury, or condition. Use this form if you have been in an accident and need to claim health benefits from your health cover.

Pre-existing condition form

Members may also need to submit this form if they have joined or upgraded their level of Hospital cover in the past 12 months. This medical report allows CBHS’ medical advisors to check whether the member’s condition is a pre-existing condition and determine whether it is covered for the member.

Claims form
